Social project support workers Roles
They are responsible for assisting clients in achieving their social, emotional, and physical well-being by implementing and participating in various social projects and programs. This role demands a deep commitment to improving the quality of life for vulnerable populations and fostering a sense of community.
Executes social projects and programmes, such as but not restricted to community outreach programmes, leisure pursuits, and educational workshops.
Establishes and builds strong bonds with individuals to ensure that their needs, preferences, and goals are recognised and satisfied.
Assess and plans
Perform in-depth analyses of the social, emotional, and physical needs of your supported persons. Creates individualised care plans that detail precise objectives and supports.
Act as advocates to vulnerable individuals by making sure their opinions are heard and their rights are upheld within the framework of the community and social care system.
Provides rapid support and aid in times of crises, emergencies, or other situations that demand immediate attention.
Documentations
Keep detailed records of supported persons encounters, progress, and outcomes. Ensures that organisational and regulatory documentation standards are met.
Collaborate with multidisciplinary teams, such as social workers, psychologists, healthcare professionals, and community organisations, to coordinate complete support for our vulnerables ones.
Coordination of Community Resources
Connect our vulnerable group with community resources such as housing, job services, mental health support, and addiction treatment etc.
Education and Guidance
Provide supported individuals and their families with guidance and educational resources to assist them in developing skills for independent living and self-sufficiency.
Monitoring and Evaluation
Constantly monitor client progress, gather data, and assess the efficacy of interventions. Change care plans as needed to get the desired results.
