Execution of Social Projects: They carry out social projects and programs, such as community outreach programs, recreational activities, and educational workshops.
Building Connections: They establish strong bonds with the supported individuals to ensure that their needs, preferences, and goals are recognised and met.
Assessment and Planning: They conduct thorough analyses of the social, emotional, and physical needs of the supported individuals. They create individualised care plans outlining specific objectives and supporting them.
Advocacy for Vulnerable Individuals: They act as advocates for vulnerable individuals, ensuring that their opinions are heard and their rights are upheld within the framework of the community and social care system.
Providing swift support in times of crisis, emergencies, or other situations requiring immediate attention.
Documentation: They maintain detailed records of encounters, progress, and outcomes of the supported individuals. They ensure that organisational and regulatory documentation standards are met.
